Complete Dental Care serves both the Adult Medicaid Patient and Medicaid Patients under the age of 21.

All patients must present a valid and active Medicaid Insurance Card along with the proper identification at each appointment.

Applicable co-pays are due at the time of each appointment.

Children must be accompanied by a parent or guardian and we must be allowed to secure a signed permission to treat each child that becomes a patient in our office. Parents are asked to remain on the premises when a child is in our care.

Physically and mentally challenged patients must be accompanied by a representative for the time that they are in our office.
